A penile implant is a device that is placed inside the penis of the man to allow him to restore his erectile function. These devices have been available since 1973 when they were first invented. Over time there have been numerous iterations of the device and improvements to the devices that we have today. These are called the inflatable penile prosthesis, which is a hydraulic device that is placed in the penis, and by squeezing a pump, it allows water to engorge the penis and develop an erection that is satisfactory for intercourse.
For the most part, as a man gets older and erectile function becomes worse, his penis will become a little bit shorter. While the main purpose of a penile implant is to allow a man to restore his erectile function, most patients will ask if the penile implant increases penis size. Most of the time, penile implants do not increase penile size. The length of the penis after penile implant placement is like the stretched penis length before surgery.
When placing an implant, the goal is to allow the man to keep the function or the length of his penis that he has at the time of the surgery. There are some surgical techniques or tricks that urologists, who specialize in sexual medicine, can do at the time of surgery to maximize the length of the penis. This includes upsizing the implant or putting in cylinders that are slightly longer than the penis. Early and aggressive cycling (inflation and deflation of the device by the patient at home) may lead to a small gain in penile length.
Through these methods we can maintain the length of the penis in the same length that it was before surgery, and in some cases are able to expand the length of the penis and allow the patient to have a little bit of length expansion. One important thing to notice though is that with the penile implant, the girth of the penis, for the most part, will become larger.
